How to Contact the Town Nurse

 

Introduction

Colonie's tradition of recognizing and meeting the health care needs of its residents dates back to the 1930's. The ongoing goal of the Town Nurse is the promotion of health and wellness, and the prevention of disease for Colonie's ever-growing population. In the face of existing or chronic disease processes, early detection, intervention and rehabilitation are employed.

The Town Nurse maintains morning office hours for the more mobile town residents. Daily hours provide the opportunity for individuals to receive blood pressure checks, small dressing changes, medication, diet and general health counseling and injections.

The Town Nurse also provides home visits to those who have health needs and are homebound or have limited transportation. Some of the nursing services provided in the home setting include: blood pressure monitoring, injections, wound care, diet and health counseling, assistance with medications, diabetic monitoring, newborn assessments, and individual/family teaching. Any resident of the Town of Colonie who is under a physician's care and has skilled nursing needs may call and request an assessment.

Frequently Asked Questions

Q. I am not a senior citizen. Am I eligible to receive services from the Town Nurse?

A. Any resident of the Town of Colonie may receive nursing services. There is no age requirement.

Q. I do not have a Doctor, can the Town Nurse take care of me instead?

A. All clients seen by the Town Nurse must be under the care of their own physician.

Q. Who must contact the Town Nurse to see if I can receive services?

A. Anyone can call the Nurse to make a referral. (Doctor's office, relative, neighbor, friend, )

Q. Is there any cost to have the Town Nurse come to my home?
A.
No. The service is free of charge to any eligible Town resident.
